Position:    Wellness Aide

 

Department:  Home and Community Based Services

Reports to:  Nurse Manager

Hours:  Part Time less than 40 hours/pp (non-benefit eligible) 4-9 shifts

     NOTE:  All positions require rotating Holidays and Every other weekend rotation

Qualifications:  Must possess current Wisconsin C.N.A. License and have ability to professionally communicate orally and in writing. Ability to work independently, be able to problem solve and demonstrate critical thinking skills.  Must have strong interpersonal communication skills to promote St. Camillus   Home services.  * Sales background is beneficial.

Responsibilities:  Assists clients with, or performs for clients, activities of daily living, including, but not limited to:

  1. Bathing/hygiene.
  2. Personal grooming; oral/denture, hair, nail, skin care.
  3. Toileting; use and care of bedpan, urinal, commode, toilet.
  4. Dressing/undressing.
  5. Ambulation/mobility; use of ambulatory devices; range of motion exercises

Maintains own schedule.  Works with clients to schedule mutually agreed-upon visit dates/times. Maintain and update daily work schedule and provide access to this schedule by all Wellness Assistants.  Report changes/concerns regarding client condition to supervisor.

 

Position: CBRF C.N.A.

 

Department:  Assisted Living

Reports to:  LPN Supervisor

Starting Range:  $12.25-14.30 (pm and night shift differentials)

Hours:  FT pm's, 2:30p-10:30pm, 60hrs, E/O weekend.  (All benefits)

    NOTE: All positions require rotating Holidays!

Responsibilities:  Primary responsibilities include providing assistance to the residents - such as, but not limited to: bathing, medication dispensation, activities, laundry, personal hygiene, etc. Assists manager in the daily operations by providing clerical assistance, etc.

Qualifications:  Must be18 years of age or older with high school diploma or GED; current CNA certification for state of WI, CBRF Certifications: Standard Precautions, Fire Safety, First Aid and choking, and Medication certificate. Good verbal & written communication skills, self-motivation, work independently with little supervision

Position: CERTIFIED NURSING ASSISTANT

 

Dept: Nursing

Reports to: Supervisor, Skilled Nursing

Hours:  FT 1st shift, 6:30 - 3:00 pm, E/O weekend  (All benefits)

    Note:  All positions require rotating holidays!

Job Description: Perform and document on direct resident care including bathing, feeding and toileting. Participate in providing restorative nursing care.

Requirements: High school grad or equivalent. Must be 18 years of age or older, on CNA registry in Wisconsin, ability to read and follow care plan. Good verbal & written communication skills, work independently with little supervision.

 

Position:  HOME HEALTH AIDE - C.N.A. PREFERRED

 

Dept:  Home Based Services

Reports to:  RN Clinical Manager-Supportive

Wage:  Varies according to qualifications

Hours:  PT, 2nd & 3rd shift available, E/O weekend & rotating holidays required  (Not Benefit Eligible)

Client visits can vary from 1-8 hours; 1 shift start time between 6am-8am, 2nd shift start time between 2pm-4pm, and 3rd shift start time between 8am-10pm.  3rd shift is for 8-12 hours.

Job Description:  Under the direction of an RN this position will:

  • Provide and/or assist clients with activities of daily living, including but not limited to bathing, grooming, toileting, mobility, meal preparations, exercises, shopping, light housekeeping, and other tasks assigned by the RN.
  • This position will provide services in the clients’ homes on campus and in the community.

Qualifications:

  • Current C.N.A. license preferred; or
  • Current C.B.R.F. certifications and prior caregiving experience
  • Current nursing student enrolled in a college nursing program
  • Previous C.N.A. license and prior caregiving experience
  • Ability to travel to and from homes within the community that may not be on the bus line;
  • When utilizing a personal vehicle, a valid license and proof of current vehicle liability insurance coverage, as well as a driving record which complies with the St. Camillus Safe Driving Policy.
  • Ability to communicate orally and in writing in a professional manner
  • Ability to maintain and safeguard entrusted confidential information
  • Customer service skills are a must!

 

Position:  CNA/ASSISTED LIVING

 

Hours:  FT pm's and night shifts available (PM's: 2:30p-10:30pm, E/O weekend)

               (Nights: 10:30pm-6:30am)

Job Description:

  • Provide assistance with activities of daily living
  • Assist the nurses and other staff as needed
  • Proper documentation per policy and procedure
  • Assist with medication administration
  • Maintains an orderly, sanitary, and safe environment

Qualifications:

  • High school diploma or GED required
  • 18yrs of age or older
  • Current C.N.A. certification
  • CBRF certifications preferred
  • Experience working with the geriatrics population preferred, especially memory care.

 

Position:  Jesuit Aide

 

Department:  Assisted Living

Reports to:  Supervisor, Assisted Living

Hours:  E/O weekend shift- 1 weekend a.m. 1 weekend p.m.

               FT, 2nd shift

     NOTE:  All positions require rotating Holidays!

Responsibilities:  This position will provide personal care and assistance to the Jesuit Community at St. Camillus. Under the supervision of the nurse, provides assistance with medication administration, laundry and cleaning duties as scheduled, Conducting scheduled and unscheduled leisure activities and perform personal care and other basic services within the guidelines established in the Care Quality Expectations

Qualifications:  Must be 18 yrs of age or older with high school diploma or GED; Current CNA certification for State of WI is required.  Current CBRF Certification is preferred but not required. Good verbal & written communication skills, self-motivated,  and  ability to work independently w/ little supervision.
