Position:  RN CASE MANAGER-HOME HEALTH

 

Dept:  Home Based Services

Reports to:  RN Clinical Manager-Home Health

Hours:  Full time (all benefits including health); hours based on client census, primarily 1st shift, with flexibility required to take 2nd shift and weekend admissions as needed

Job Summary:  This position will utilize safe clinical judgment and assessment skills, coordinate, implement, and evaluate plans of care for home health clients served according to regulations as well as proved high quality nursing care.  This position will also be responsible for communicating with client family members and other members of the interdisciplinary team.

Qualifications:

  • Current WI Registered Nurse License (BSN preferred) as well as current CPR certification.
  • Minimum of 3 years medical-surgical experience as a an RN, 2+ years Home Health experience preferred.
  • Strong and professional verbal and written communication skills.
  • Prior experience with electronic documentation skills as well as the ability to meet visit documentation submission deadlines.
  • Time management and customer service skills helpful.
  • When utilizing a personal vehicle to travel to client homes, a valid WI Driver’s License, proof of auto insurance, and a driving record which complies with the St. Camillus Safe Driving policy.

 

Position:  RN UNIT MANAGER

 

Department:  Skilled Nursing

Reports to:  Director of Nursing

Hours:  FT, 1st shift, Mon-Fri, 80 hrs/pp; Occasionally other shifts, weekends, holidays

Responsibilities:  In conjunction with other members of nursing leadership team, will oversee care & services for residents in our 50-bed Skilled Nursing Facility.  Has accountability for administrative & clinical issues, supervising staff to ensure residents receive excellent and timely care.  Will conduct admissions assessment and documentation, MDS assessments, care plan documentation, and discharge planning.

Requirements:  Current WI RN license & minimum of 5 yrs supervisory experience in a skilled nursing facility preferred, thorough knowledge of state and federal regulations, strong computer data entry skills.  Current CPR certification.

 

Position:  LPN

 

Dept:   Skilled Nursing

Reports to:  Director of Nursing

Hours:   FT days, 64 or 46hrs/bi-weekly, E/O weekend

                   PT day's, 6:30a-3pm, E/O weekend,

                   Pool positions available: $27/HR Requirements: 2 weekend shifts/month & 2 Holidays/yr. All shifts available for pool.

Job Description:  Provides skilled nursing care and assists RN to meet the physical, spiritual and psychosocial needs of the residents.  The staff LPN responds in emergency situations to assure resident safety and security.

Requirements:   Current WI LPN license, graduate of an accredited school of nursing.  Minimum  2 years professional nursing experience, preferably experience in a LTC facility.

 

Position:  RN

 

Dept:   Skilled Nursing

Reports to:  Director of Nursing

Hours: FT days, 56 & 64hrs/bi-weekly, E/O weekend.

                 PT day's, 6:30a-3pm, bi-weekly.

                 PT pm's, 2:30p-11pm, E/O weekend

                 POOL POSITIONS: $35/HR; Requirements: 2 weekend shifts/month & 2 Holidays/yr.

    NOTE: All positions require rotating holidays and E/O weekend.

Responsibilities:  Provides skilled nursing care and assists to meet the physical, spiritual and     psychosocial needs of the residents.  The staff RN responds in emergency situations to assure resident safety and security. No MDS Responsibilities.

Qualifications:   Current WI RN license, graduate of an accredited school of nursing.  Minimum  2 years professional nursing experience, preferably experience in a LTC facility. Current CPR license.

 

Position:  RN SUPERVISOR

 

Department:  Skilled Nursing: LTC/Rehab

Reports to:  Director of Nursing

Salary:  $37/HR for pool

Hours:  Pool Requirements: 2 weekend shifts/month & 2 Holidays/yr. All shifts available.

Responsibilities:  Oversees nursing care provided on the shift including assisting nurse managers in evaluating staff, assessing residents, making daily rounds, delegating assignments to staff and other duties as assigned.

Requirements:  Current WI RN license, 2+yrs related experience preferred. Strong clinical/supervisory skills & knowledge of federal/state regulations, accurate data entry skills and familiarity with electronic health records. Current CPR certification.

 

Position:  LPN SUPERVISOR/ASSISTED LIVING (10/11)

 

Hours:  Pool positions open available: Requirements: 2 weekend shifts/month & 2 Holidays/yr. All shifts available for pool.

Job description:

  • Provides support and oversight to Residential Aides, I.e., coaching, evaluations, communication regarding resident care
  • Provides assistance with medications, sets up, follow up with Physician, conduct audits
  • Assists in treatments and services, reports and documents appropriately
  • Assists in the resident services plans, attends weekly meetings

Requirements:

  • Graduate of an accredited school of nursing, Current State of WI licensure as an LPN
  • Community-Based Residential Facilities (CBRF)-certified as per State of WI Administrative Code DHS 83 requirements
  • Current CPR Certification
  • Ability to work independently with minimal supervision
