Position:  RN SUPERVISOR

 

Department:  Skilled Nursing

Reports to:  Director of Nursing

Hours:  PM's & noc's (combined)  60hrs/bi-weekly, E/O weekend, Pm's= 2:30p-11p,  Noc's= 10:30p-7am,  (Benefit eligible for all benefits)

Responsibilities:  Oversees nursing care provided on the shift including assisting nurse managers in evaluating staff, assessing residents, making daily rounds, delegating assignments to staff and other duties as assigned.

Requirements:  Current WI RN license, 2+yrs related experience preferred.  Strong clinical/supervisory skills & knowledge of federal/state regulations, accurate data entry skills and familiarity with electronic health records.  Current CPR certification.

 

Position:  Health Care Senior Living Specialist  (Admissions Rep)   (10/9/18)

 

Department:  Marketing

Reports to:  Director of Marketing

Hours:  FT, 1st shift

Responsibilities:  Responsible for the efficiently processing of all hospital referrals to our Skilled Nursing Facility, including conducting tours, obtaining appropriate authorizations and completion of paperwork. Supports Marketing efforts for all programs and services of St. Camillus Life Plan Community.

Qualifications:  Graduate of accredited school of nursing or related field.  Possess strong communication and critical thinking skills related to the elderly.  Ability to work in a fast paced environment and effectively multitask.  Strong organization skills and attention to detail are critical.  Ability to function well in a team environment. Understanding and ability to comply with Federal and State rules and regulations as it applies to Nursing Homes.

 

Position:  RN CASE MANAGER-HOME HEALTH

 

Dept:  Home Based Services

Reports to:  RN Clinical Manager-Home Health

Hours:  Full time (eligible for health insurance); hours based on client census, primarily 1st shift visits, with flexibility required

Job Summary:  This position will utilize safe clinical judgment and assessment skills, coordinate, implement, and evaluate plans of care for home health clients served according to regulations as well as proved high quality nursing care.  This position will also be responsible for communicating with client family members and other members of the interdisciplinary team.

Qualifications:

  • Current WI Registered Nurse License (BSN preferred) as well as current CPR certification.
  • Minimum of 3 years medical-surgical experience as an RN, 2+ years Home Health experience preferred.
  • Strong and professional verbal and written communication skills.
  • Prior experience with electronic documentation skills as well as the ability to meet visit documentation submission deadlines.
  • Time management and customer service skills helpful.
  • When utilizing a personal vehicle to travel to client homes, a valid WI Driver’s License, proof of auto insurance, and a driving record which complies with the St. Camillus Safe Driving policy.

 

Position:  RN ON CALL

 

Dept:  Home Based & Community Services

Reports to:  RN Clinical Manager-Hospice

Hours:  1 week per month after business hours only (non-benefit eligible)

Job Summary:  This position works from home answering after business hour nursing related calls for our Home Health & Hospice departments.  Availability to triage situations and provide client visits as necessary is required.  The position allows professional freedom, while providing a high level of nursing care.

Qualifications:

  • Current WI RN License; BSN preferred
  • Minimum of 1 year professional experience, preferably in a Home Health and/or Hospice Case management setting.
  • When utilizing a personal vehicle to travel to client homes, a valid WI Driver’s License, proof of auto insurance, and a driving record which complies with the St. Camillus Safe Driving policy.

 

St. Camillus has an opening in our GROWING Joint Commission -accredited Home Health & Supportive agency for an experienced, compassionate, and self-directed RN Clinical Manager to lead a team of talented RNs, LPNs, C.N.A.s, and Supportive Aides.  This position is a salaried, full time position, primarily 1st shift with flexibility according to team and client need.  This position is also eligible for a generous benefits and compensation package.

  • Minimum of 5 years Home Health experience and an understanding of Home Health guidelines including OASIS, COPs, coding, and reporting practices.
  • Strong individual and group presentation skills as well as strong interpersonal communication skills.
  • Previous experience supervising case management, client relations and concerns, and employee relations and concerns.
  • Previous budgeting experience helpful.
  • Valid WI Driver’s license, auto insurance, and a driving record which complies with the St. Camillus Safe Driving Policy.

 

Position:  LPN

 

Dept:   Skilled Nursing

Reports to:  Director of Nursing

Hours:   PT days, 48hrs/bi-weekly, E/O Weekend, 6:30a-3pm.

                   PT pm's, 56hrs/bi-weekly, E/O weekend, 2:30p-11pm.

                   PT noc's, 40hrs/bi-weekly, E/O weekend,  10:30p-7am.

                   Pool positions available: $27/HR Requirements: 2 weekend shifts/month & 2 Holidays/yr. All shifts available for pool.

Job Description:  Provides skilled nursing care and assists RN to meet the physical, spiritual and psychosocial needs of the residents.  The staff LPN responds in emergency situations to assure resident safety and security.

Requirements:   Current WI LPN license, graduate of an accredited school of nursing.  Minimum  2 years professional nursing experience, preferably experience in a LTC facility.

 

Position:  RN

 

Dept:   Skilled Nursing

Reports to:  Director of Nursing

Hours:  Noc's, E/O weekend, 16hrs/bi-weekly, 10:30p-7am.  Shift differential of $1.25/HR and guaranteed E/O wknd plus pick up shifts at premium rate.

                  POOL POSITIONS: $35/HR; Requirements: 2 weekend shifts/month & 2 Holidays/yr.

    NOTE: All positions require rotating holidays and E/O weekend.

Responsibilities:  Provides skilled nursing care and assists to meet the physical, spiritual and     psychosocial needs of the residents.  The staff RN responds in emergency situations to assure resident safety and security. No MDS Responsibilities.

Qualifications:   Current WI RN license, graduate of an accredited school of nursing.  Minimum  2 years professional nursing experience, preferably experience in a LTC facility. Current CPR license.

 

Position:  RN SUPERVISOR

Reports to:  Director of Nursing

Salary:  $37/HR for pool

Hours:  Pool Requirements: 2 weekend shifts/month & 2 Holidays/yr. All shifts available.

                  Pool positions available: Requirements:  2 weekend shifts/month  & 2 Holidays/yr.   All shifts available for pool.

Job description:

  • Provides support and oversight to Residential Aides, I.e., coaching, evaluations, communication regarding resident care
  • Provides assistance with medications, sets up, follow up with Physician, conduct audits
  • Assists in treatments and services, reports and documents appropriately
  • Assists in the resident services plans, attends weekly meetings

Requirements:

  • Graduate of an accredited school of nursing, Current State of WI licensure as an LPN
  • Community-Based Residential Facilities (CBRF)-certified as per State of WI Administrative Code DHS 83 requirements
  • Current CPR Certification
  • Ability to work independently with minimal supervision
