Position:  LPN

 

Dept:   Skilled Nursing

Reports to:  Director of Nursing

Hours:   FT days, 80hrs/bif-weekly, E/O weekend

                   PT day's, 6:30a-3pm, E/O weekend, 16hrs/bi-weekly

                   PT pm's, 2:30p-11pm, E/O weekend, 16hrs/bi-weekly

                   PT pm's, 2:30-11pm, E/O weekend, 40hrs/bi-weekly

                   Pool positions available: $27/HR Requirements: 2 weekend shifts/month & 2 Holidays/yr. All shifts available for pool.

Job Description:  Provides skilled nursing care and assists RN to meet the physical, spiritual and psychosocial needs of the residents.  The staff LPN responds in emergency situations to assure resident safety and security.

Requirements:   Current WI LPN license, graduate of an accredited school of nursing.  Minimum  2 years professional nursing experience, preferably experience in a LTC facility.

Reports to:  Director of Nursing

                 POOL POSITIONS: $35/HR; Requirements: 2 weekend shifts/month & 2 Holidays/yr.

NOTE: All positions require rotating holidays and E/O weekend.

Responsibilities:  Provides skilled nursing care and assists to meet the physical, spiritual and     psychosocial needs of the residents.  The staff RN responds in emergency situations to assure resident safety and security. No MDS Responsibilities.

Qualifications:   Current WI RN license, graduate of an accredited school of nursing.  Minimum  2 years professional nursing experience, preferably experience in a LTC facility. Current CPR license.

 

Position:  RN SUPERVISOR

 

Department:  Skilled Nursing: LTC/Rehab

Reports to:  Director of Nursing

Salary:  $37/HR for pool

Hours:  Pool Requirements: 2 weekend shifts/month & 2 Holidays/yr. All shifts available.

Responsibilities:  Oversees nursing care provided on the shift including assisting nurse managers in evaluating staff, assessing residents, making daily rounds, delegating assignments to staff and other duties as assigned.

Requirements:  Current WI RN license, 2+yrs related experience preferred. Strong clinical/supervisory skills & knowledge of federal/state regulations, accurate data entry skills and familiarity with electronic health records. Current CPR certification.

 

Position:  LPN SUPERVISOR/ASSISTED LIVING (10/11)

 

Hours:  FT pm's, 80hrs/bi-weekly, E/O weekend. (All benefits)

                  PT noc's, 48hrs/bi-weekly, E/O weekend.  (All benefits except health)

                  Pool positions open available: Requirements: 2 weekend shifts/month & 2 Holidays/yr. All shifts available for pool.

Job description:

  • Provides support and oversight to Residential Aides, I.e., coaching, evaluations, communication regarding resident care
  • Provides assistance with medications, sets up, follow up with Physician, conduct audits
  • Assists in treatments and services, reports and documents appropriately
  • Assists in the resident services plans, attends weekly meetings

Requirements:

  • Graduate of an accredited school of nursing, Current State of WI licensure as an LPN
  • Community-Based Residential Facilities (CBRF)-certified as per State of WI Administrative Code DHS 83 requirements
  • Current CPR Certification
  • Ability to work independently with minimal supervision

 

Position: RN CASE MANAGER-HOSPICE

 

Dept: Home Based Services

Reports to: RN Clinical Manager-Hospice

Hours: Full time (all benefits); hours based on client census, primarily 1st shift, does require 1 week/month rotating on call hours during 2nd and 3rd shift

Job Summary: This position will utilize clinical judgment and assessment skills, coordinate, implement, and evaluate plans of care for hospice clients served according to Hospice regulations. The position will also be responsible for communicating with client family members and other members of the interdisciplinary team.

Qualifications:

  • Current WI Registered Nurse License (BSN preferred) as well as current CPR certification.
  • Minimum 3 years medical-surgical experience as an RN, 2+ years Hospice experience preferred.
  • Strong and professional verbal and written communication skills.
  • Prior electronic documentation skills preferred as well as the ability to meet visit documentation submission deadlines.
  • Time management and customer service skills helpful.
  • Ability to educate and promote Hospice services effectively.
  • When utilizing a personal vehicle to travel to client homes, a valid WI Driver’s License, proof of auto insurance, and a driving record which complies with the St. Camillus Safe Driving policy.

 

Position:  LPN-VISIT NURSE

 

Department:  Home Based Services

Reports to:  RN Clinical Manager-Home Health

Hours:  Per Visit; hours are based on client census, mainly 1st shift hours (self- schedule client visits per the RN Case Managers)

Job Description:  Provides nursing services, delegated by the RN Case Managers or the RN Clinical Manager to clients on campus and off site in their homes in the community.

Qualifications:

  • Current WI LPN license and current CPR certification.
  • Minimum 1 year medical surgical experience as an LPN.
  • Ability to see, write, hear, and speak effectively.
  • Previous Home Health nursing experience a plus.
  • Current WI Driver’s license, current auto insurance, and a driving record which complies with the St. Camillus Safe Driving policy

 

Position:  RN CASE MANAGER-HOME HEALTH

 

Dept:  Home Based Services

Reports to:  RN Clinical Manager-Home Health

Hours:  Full time (all benefits including health); hours based on client census, primarily 1st shift, with flexibility required to take 2nd shift and weekend admissions as needed

Job Summary:  This position will utilize safe clinical judgment and assessment skills, coordinate, implement, and evaluate plans of care for home health clients served according to regulations as well as proved high quality nursing care.  This position will also be responsible for communicating with client family members and other members of the interdisciplinary team.

Qualifications:

  • Current WI Registered Nurse License (BSN preferred) as well as current CPR certification.
  • Minimum of 3 years medical-surgical experience as a an RN, 2+ years Home Health experience preferred.

Position:  Hospice Clinical Manager

 

Department:  Home and Community Based Services

Reports to:  Administrator, HCBS

Hours:  Day Shift Hours but must be flexible, based on client needs.

NOTE:  All positions require rotating Holidays!

Responsibilities:  Coordinates the day-today activities of the Hospice Department to ensure the delivery of quality, comprehensive healthcare services.

Quality:  Ensures compliance with all professional standards and applicable state, federal and Join Commission regulations and agency policies. Ensures compliance with reporting practices.

Supervisory:  Coaches, counsels and evaluates performance of Hospice clinical staff. Provides initial and ongoing orientation/training for Hospice and on-call staff.

Communication:  Serves as resource to staff regarding client care issues, policies/procedures, and regulations. Participates in formal (IDG) and informal case conferences.

Day to Day Operations:  Assigns admissions to RN Case Managers. Coordinates daily nursing visits with Case Managers and other staff. Monitors client care supplies. Performs duties of Case Manager when necessary. Coordinates referrals with Intake Department.

Qualifications:  Current WI RN License. BSN preferred; Hospice Certification preferred. Minimum 2 years of supervisory experience. Minimum of 5 years Hospice experience. Previous budgeting experience helpful. Understanding of Hospice guidelines, including COPs, coding and reporting practices. Must be flexible, organized and have the ability to change directions quickly. Strong computer skills, with previous experience using Hospice clinical software a plus.
